Case overview
This case shows an implant placed at the lower left second molar (#37). Mandibular molar sites carry a mix of dense cortical bone at the crest and trabecular bone in the deeper portion of the osteotomy, and are among the most common indications for posterior implant placement.
The Crown Down 2-drill protocol is designed for exactly this anatomical setup. The cortical drill matched to implant diameter clears the crestal cortex in a single controlled pass. A smaller trabecular drill then prepares the deeper cancellous portion. The osteotomy is fully prepared in two passes rather than the five-to-eight-drill ascending sequence a conventional stainless-steel kit would use.
Because the drills are solid tungsten carbide, they hold their cutting edge under the crestal cortical load and conduct heat away from the cutting interface. The full osteotomy is prepared at low rotational speed without continuous external irrigation, and the implant is seated at controlled depth using the coded depth-stopper system.
